Our caregivers not only possess the necessary training to address the specific challenges faced by medically fragile children, but many are also family members who deeply understand the importance of personalized and nurturing care. Thanks to the Georgia Pediatric Program (GAPP), caregivers can receive compensation through Medicaid, enabling families to focus on what truly matters: the health and happiness of their children.
In collaboration with HealthySteps Interventions, we ensure that our caregivers can access the resources and training required to offer exceptional care. We strive to create a seamless experience for both caregivers and families, fostering a supportive network that empowers everyone involved in this vital caregiving journey.
Caregivers are required to undergo a background check. There are no income restrictions for individuals seeking to become family caregivers.
Nurse case management involves a registered nurse serving as a coordinator and advocate for patients throughout their healthcare journey. Community-based case management involves providing coordinated and individualized support and services to individuals in a community setting. This approach focuses on assessing clients' needs, developing personalized care plans, coordinating access to resources and services, monitoring progress, and advocating for clients' well-being. Community-based case managers work collaboratively with clients, their families, and community organizations to promote stability, independence, and overall improved quality of life. The goal is to address clients' needs holistically and empower them to achieve their goals and thrive in their community.
Parent mentorship involves providing guidance, support, and resources to parents by experienced and knowledgeable individuals who have gone through similar experiences raising children. The goal is to empower parents with the skills and knowledge needed to navigate challenges, develop healthy parenting practices, and promote positive outcomes for their children. Parent mentors offer emotional support, practical advice, and encouragement, serving as a role model and advocate for parents in their journey of raising children. The relationship is built on mutual trust, respect, and understanding, with the ultimate objective of helping parents build strong and nurturing relationships with their children.
