Our Services

  • Our agency offers a dedicated platform that empowers family caregivers to provide vital care for medically fragile children right in their homes. We are committed to supporting families by connecting them with compassionate and qualified caregivers who specialize in the unique medical needs of these children and can provide daily assistance.

Our caregivers not only possess the necessary training to address the specific challenges faced by medically fragile children, but many are also family members who deeply understand the importance of personalized and nurturing care. Thanks to the Georgia Pediatric Program (GAPP), caregivers can receive compensation through Medicaid, enabling families to focus on what truly matters: the health and happiness of their children.

In collaboration with HealthySteps Interventions, we ensure that our caregivers have access to the resources and training required to offer exceptional care. We strive to create a seamless experience for both caregivers and families, fostering a supportive network that empowers everyone involved in this vital caregiving journey.

Caregivers are required to undergo a background check. There are no income restrictions for individuals seeking to become family caregivers.

  • Nurse case management services typically include:

    1. Initial assessment of the child’s health needs and goals

    2. Development of a care plan tailored to the individual child’s needs

    3. Coordination of healthcare services and resources

    4. Monitoring and evaluation of the patient's progress

    5. Regular communication with the patient, their family, and healthcare providers

    6. Advocacy on behalf of the child to ensure they receive appropriate care

    7. Support and education for the child and their family on health-related issues

    8. Assistance with navigating the healthcare system and accessing community resources.

  • Community-based case management services can vary depending on the specific needs of the children and families being served. However, some common services provided by community-based case managers may include:

    1. Assessment of the child’s needs, strengths, and challenges

    2. Development of a comprehensive care plan in collaboration with the individual and their support network

    3. Referral to and coordination with appropriate community resources and services, such as housing assistance, mental health support, substance abuse treatment, and financial assistance programs

    4. Advocacy on behalf of the child to ensure they receive needed services and support

    5. Monitoring and evaluation of the child’s progress towards their goals

    6. Crisis intervention and support during times of heightened need or emergency

    7. Education and skill-building to empower individuals to navigate systems and resources independently

    8. Regular communication and collaboration with other service providers, such as healthcare professionals, social workers, and educators.

These services are designed to support individuals in achieving their goals, improving their quality of life, and enhancing their overall well-being within their community.

  • Parent mentorship services are typically designed to provide support, guidance, and resources to parents who may be facing challenges or seeking assistance in navigating the complexities of raising children with medical challenges. Some common parent mentorship services may include:

    1. One-on-one mentoring: Matching parents with experienced mentors who provide support, guidance, and a listening ear on a personalized basis.

    2. Parenting workshops and classes: Offering group sessions that focus on specific parenting topics, such as discipline, communication, and managing stress.

    3. Support groups: Providing a safe and supportive environment for parents to connect with others facing similar challenges, share experiences, and receive emotional support.

    4. Child development resources: Offering information and guidance on developmental milestones, age-appropriate activities, and strategies for promoting positive child outcomes.

    5. Referral services: Connecting parents with community resources, such as childcare assistance, mental health services, and educational programs.

    6. Advocacy: Supporting parents in navigating systems, advocating for their children's needs, and accessing appropriate services and supports.

    7. Parent education: Providing information and resources on parenting techniques, child behavior, and family dynamics.

    8. Skill-building: Offering opportunities for parents to develop skills in areas such as effective communication, problem-solving, and stress management.

Overall, parent mentorship services aim to empower parents with the knowledge, skills, and resources they need to foster healthy and positive relationships with their children, navigate challenges, and promote their children's well-being and development.
